2.A. (U) SERGEANT FIRST CLASS (SFC) BRENDA PHYONA ARRINDELL SERVED
HER COUNTRY FOR OVER 18 YEARS. SHE REPRESENTED THE HIGHEST
IDEALS OF THE U.S. ARMY MEDICAL DEPARTMENT (AMEDD) AND RECEIVED
ACCOLADES DURING HER CAREER. ON 2 FEBRUARY 2011, SFC ARRINDELL
DIED SUDDENLY AFTER PHYSICAL TRAINING WHILE SERVING AS ONE OF THE
LEAD INSTRUCTORS AT THE NONCOMMISSIONED OFFICER (NCO) ACADEMY IN
JOINT BASE SAN ANTONIO–FORT SAM HOUSTON, TX. THE SFC BRENDA
PHYONA ARRINDELL AWARD IS AN ANNUAL AWARD GIVEN TO AN
OUTSTANDING NCO IN THE PATIENT ADMINISTRATION CAREER FIELD (68G).
THE WINNER OF THE ARRINDELL AWARD EMULATES THE HIGHEST
STANDARDS ESTABLISHED BY SFC BRENDA PHYONA ARRINDELL AND IS A
CREDIT TO THE CAREER FIELD, THE AMEDD, AND THE U.S. ARMY.
2.B. (U) LIEUTENANT COLONEL (LTC) PAUL HENRY HATKOFF SERVED HIS
COUNTRY FOR OVER 22 YEARS. HE SERVED HONORABLY AND WAS HIGHLY
DECORATED THROUGHOUT HIS CAREER. HE REPRESENTED THE HIGHEST
IDEALS OF THE PATIENT ADMINISTRATION DIRECTOR (PAD) FIELD, THE
MEDICAL SERVICE CORPS, AND THE AMEDD IN EVERY ENDEAVOR. ON 14
APRIL 1991, LTC HATKOFF DIED SUDDENLY WHILE JOGGING. AT THE TIME OF
HIS DEATH, HE SERVED AS THE CHIEF, PAD OF WILLIAM BEAUMONT ARMY
MEDICAL CENTER AT FORT BLISS, TEXAS. HE WAS A GRADUATE OF THE U.S.
ARMY BAYLOR PROGRAM IN HEALTHCARE ADMINISTRATION. THE LTC PAUL
HENRY HATKOFF AWARD IS AN ANNUAL AWARD GIVEN TO AN OUTSTANDING
COMPANY GRADE OFFICER IN THE PAD CAREER FIELD. THE WINNER OF THE
HATKOFF AWARD EMULATES THE HIGHEST STANDARDS ESTABLISHED BY LTC
PAUL HENRY HATKOFF AND IS A CREDIT TO THE PAD CAREER FIELD, THE
AMEDD, AND THE U.S. ARMY.
2.C. (U) MRS. TERESA (TERRY) FOLEY SERVED AS A DISTINGUISHED HEALTH
INFORMATION MANAGEMENT (HIM) EXPERT FOR OVER 40 YEARS. SHE
SERVED AS A HIM DIRECTOR IN SEVERAL CIVILIAN AND MILITARY HOSPITALS.
IN HER FINAL AMEDD POSITION, MRS. FOLEY SERVED AS THE MEDICAL
RECORDS ADMINISTRATOR FOR THE OFFICE OF THE SURGEON GENERAL. IN
THAT POSITION SHE SERVED AS THE LEAD FOR HEALTHCARE
DOCUMENTATION AND MEDICAL RECORD ADMINISTRATION. MRS. FOLEY
RECOGNIZED THE NEED FOR PAD OFFICERS TO RECEIVE SPECIFIC TRAINING
ON HIM AND COORDINATED WITH THE OFFICE OF THE CHIEF OF THE MEDICAL
SERVICE CORPS TO DEVELOP THE LONG–TERM HEALTH EDUCATION AND
TRAINING PROGRAM AT THE UNIVERSITY OF PITTSBURGH. THE TERESA
FOLEY AWARD IS ESTABLISHED TO RECOGNIZE DEPARTMENT OF DEFENSE
CIVILIAN EMPLOYEES’ CONTRIBUTIONS TO THE CONTINUED SUCCESS OF THE
PAD MISSION. THE WINNER OF THE FOLEY AWARD EMULATES THE HIGHEST
STANDARDS ESTABLISHED BY MRS. TERESA FOLEY AND IS A CREDIT TO THE
CIVILIAN CAREER FIELD, THE AMEDD, AND THE U.S. ARMY.
3. (U) ELIGIBILITY/SELECTION CRITERIA FOR THE AWARDS:
3.A. (U) SFC ARINDELL AWARD: ACTIVE, GUARD, OR RESERVE COMPONENT
AMEDD NCO IN THE RANK OF SERGEANT TO SERGEANT FIRST CLASS.
INDIVIDUALS SHOULD HOLD THE 68G MILITARY OCCUPATIONAL SPECIALTY OR
CURRENTLY WORK IN A PATIENT ADMINISTRATION POSITION.
3.B. (U) LTC HATKOFF AWARD: ACTIVE, GUARD, OR RESERVE COMPONENT
AMEDD COMMISSIONED OFFICER IN THE RANK OF SECOND LIEUTENANT TO
CAPTAIN. INDIVIDUALS SHOULD HOLD THE 70E AREA OF CONCENTRATION OR
CURRENTLY WORK IN A PATIENT ADMINISTRATION POSITION.
3.C. (U) TERESA FOLEY AWARD: CIVILIAN STAFF IN THE GRADE OF GS–04 TO
GS–13 CURRENTLY WORKING IN A PAD POSITION.
3.D. (U) NOMINEES (SOLDIERS) MUST NOT BE FLAGGED OR UNDER UCMJ
ACTIONS IN ACCORDANCE WITH REFERENCE 1.A.
